Instructions

  4. Bring the unsigned document to a notary public. Sign in front of the notary.
  5. Your agent must also sign before a notary public — this can be done at a different time.
  6. Keep the original in a safe place. Give copies to your agent, your bank, and any relevant institutions.
  7. To revoke the POA, provide written notice to your agent and any third parties who acted on it.
